Description
Harrison HealthPartners Forks Family Medical Center provides a full range of primary care services for children and adults. We serve all patients in Forks and the surrounding communities and our clinical staff is fluent in Spanish.
Our Electronic Medical Records will improve patient care by electronically sending prescriptions to your pharmacy, providing medical information to any physician within the Harrison Medical Center system, and accepting lab results from the Forks Community Hospital. Access to Harrison HealthPartners specialty services will minimize the need to travel to Seattle for healthcare services.
We also offer a Sliding Fee Scale for our services in Forks for patients who meet financial eligibility requirements.

Making an appointment
When making an appointment, please inform us of:
- Your primary care/referring provider
- Your primary/secondary insurance carrier
- Which practitioner you wish to see
- The reason for the visit or health issue you are most concerned about
We appreciate receiving 24 hours notice if you are unable to keep your appointment. This will allow us to give another patient the opportunity to be seen at that time.
